Job summary

ScionHealth is seeking a skilled Care Coordination professional to support the Interdisciplinary Care Transitions (ICT) team. You will coordinate and facilitate patient care across the continuum, ensuring optimal resource use, service delivery, and compliance with regulatory reviews.

Responsibilities include comprehensive assessments, care planning, plan implementation, and ongoing evaluation to enhance patient management and satisfaction while integrating case management, utilization review,

Qualifications

• Graduate of an accredited program required: LPN/LVN or RN.
• Master of Social Work with licensure as required by state regulations or Bachelor of Social Work with licensure as required by state regulations.
• Healthcare professional licensure required as LPN/LVN, Registered Nurse, or LCSW or LSW if required by state regulations.

Responsibilities

• Coordinate clinical and psycho-social activities with the Interdisciplinary Team and Physicians.
• Assist with effective care coordination and efficient care facilitation.
• Refer high risk patients for additional support as needed.
• Serve as a patient advocate.
• Participate in interdisciplinary patient care rounds and/or conferences.
• Discharge planning under supervision and comprehensive assessment of patients and families.
