Job summary

ScionHealth in Las Vegas is seeking a dedicated case management SME to lead care coordination across our hospitals. You will mentor staff, educate on discharge planning, resource utilization, and psychosocial aspects of care while ensuring compliance with CMS, state, and accreditation standards.

Qualifications

• RN, BSN preferred; or Master of Social Work with licensure as required by state regulations.
• Healthcare licensure required per state regulations.

Responsibilities

• Care Coordination: expert daily coordination of patient care to develop, monitor, and refine treatment plans.
• Discharge Planning: comprehensive ongoing assessment to provide timely and safe discharge planning.
• Utilization Management: review medical necessity and optimize clinical resource utilization.
• Other: comply with Code of Conduct and participate in hospital committees.
