Job summary

Trinity Health in Ohio is seeking a Senior Coordinator, Case Management & Social Services to provide administrative and operational support to Case Managers and Social Workers in a fast-paced acute care hospital. You will assist with discharge planning, coordination of post-acute services, documentation, insurance and referrals, and ensure timely follow-up while maintaining excellent customer service.

Qualifications

• Associate's degree or High School Diploma and equivalent experience required.
• Bachelor's degree preferred.
• Medical assistant or Licensed Practical Nurse (LPN) highly preferred.
• 5-7 years of customer service, Insurance knowledge medical assistance or secretarial experience preferred.
• Prior experience in a medical setting required.
• Ability to organize and utilize work hours effectively and with minimal supervision.
• Medical terminology is a must.
• Must be able to use Outlook, EPIC, Teams, Faxing, and phone knowledge.

Responsibilities

• Communicate information received from payers to utilization review nurse.
• Transmit continued stay reviews and track authorizations
• Complete HENS/PASSR
• Scheduling PCP/follow up appointments
• Faxing and phoning agencies and facilities to assist with discharge referrals and continuity of care
• Assist with delivery of charity items-clothing/DME/meal cards, etc. and complete transportation application and arrange transportation as needed for patients at discharge
• Completing PCS forms
• Compiling packets for transport
• DME: Completes form, get the script, and call liaison to follow up to ensure the patient receives before leaving.
• Sending referrals through Epic CareLink, Facilities, and IV infusion
• Calling facilities checking on patient acceptance
• Faxing paperwork to facilities
• Printing documents from Epic
• Problem Solving and Multi-tasking is a must quality
• Build relationships with Community Partners
• This is a full-time position at Mount Carmel St. Ann's and supports the North Region to include MCNA and MCD.
