Job summary

Morgan’s Wonderland seeks an Established Navigator to maintain long‑term engagement with MAC Members and families, ensuring continuity of the MAC Care Plan and access to services. This center-based role supports individuals with special needs, coordinating referrals and sustaining a stable support network.

The navigator reviews the care plan, communicates with families, and ensures a smooth transition when re-engagement or reassessment is needed, while upholding confidentiality and service

Qualifications

• Bachelor’s or advanced degree in a social, behavioral, human service, or health-related field; or high school diploma with qualifying experience.
• At least two years of related experience working with individuals with special needs or vulnerable populations.
• Knowledge of Medicaid, Social Security, healthcare, and community supports.

Responsibilities

• Established Member relationship and transition management.
• Proactive outreach to maintain active services and availability.
• Evaluate emerging needs and determine if maintenance or active Navigation is required.
• Document contacts, outcomes, and rationale for continuing established status or re-engaging Navigation.
• Coordinate with Members, families, providers, and community partners to preserve supports.
