The Established Navigator role at The Multi-Assistance Center At Morgans Wonderland focuses on maintaining long-term Member engagement within the MAC Care Plan. The Navigator handles routine maintenance, coordinates referrals, and supports transitions back to active Navigation when reassessment or intensified coordination is required.

Qualifications

• Bachelor’s or advanced degree from an accredited college or university; or an associate degree in a social, behavioral, human service, or health-related field; or a high school diploma or state-recognized equivalent with appropriate qualifying experience.
• At least two years of related experience working with individuals with special needs or vulnerable populations with chronic or complex conditions.
• Must present a neat, clean, well-groomed appearance and have a valid Texas driver’s license with reliable transportation.

Responsabilités

• Established Member relationship and transition: accept assigned Members, review the MAC Care Plan, and ensure smooth handoff.
• Proactive outreach and continuity: conduct planned outreach and verify services remain active and useful.
• Emerging needs and active Navigation re-engagement: decide if maintenance is enough or if active Navigation is needed; arrange referrals.
• Documentation, advocacy, and service excellence: document contacts and outcomes, coordinate with providers, and support leadership in resolving issues.
