Job summary

Access Community Health Network (ACCESS) is a mission-driven organization improving community health across local and national levels. Our team supports referrals, authorizations, and patient coordination within a comprehensive care model, driving health equity for over 150,000 patients annually.

The Referral Coordinator role requires at least 1 year in a clinical office or health care setting, along with basic proficiency in Microsoft Office and a bilingual English/Spanish advantage.

Qualifications

• High school diploma or GED required.
• 1-year experience in a clinical office or health care environment in patient registration, referrals, medical assisting or customer service required.
• Basic proficiency with Microsoft Office Suite (Word, Excel, Outlook) beneficial.

Responsibilities

• Process referrals/authorizations with Payors/Plans per policy and procedure.
• Assist providers/staff in identifying in-network providers for referred services.
• Assist patients with questions regarding referrals.
• Coordinate with departments to schedule appointments and provide necessary information.
• Monitor open referrals and obtain consultation notes from providers and notify patients to ensure referred services are received.
