Qualifications

• Associates degree or 5 years health care/hospice experience required; Bachelor's preferred.
• Minimum 2 years telephone-related experience; health care environment preferred.
• Strong proficiency with Microsoft Office, especially Excel; data entry accuracy.
• Excellent interpersonal skills, judgement, organization, and confidentiality.

Responsibilities

• Handle referrals via phone, fax, email and ECIN as first contact for hospice care.
• Provide courteous, knowledgeable information and a welcoming environment.
• Coordinate with internal departments to support patient admissions and care.
• Enter referral data accurately and adapt to changing workload and priorities.
• Maintain compliance with policies, procedures and regulatory requirements.
• Communicate effectively with families, clinicians, and administration.
