Job summary

The role requires RN licensure in Michigan and 2+ years hospice or healthcare sales experience, with strong communication and relationship-building skills and proficiency in Microsoft Office. Travel and scheduling flexibility are expected.

• Must be able to read, write and speak English and communicate effectively in internal and external relationships.
• Ability to effectively use technology in support of clinical operations, including Microsoft Office.
• Excellent interpersonal skills, sound judgment, organizational and follow-through abilities.

Responsibilities

• Develops and sustains strong relationships with referral sources to increase referrals and admissions.
• Develops, implements and takes responsibility for an annual hospital plan to increase referrals and conversion rate.
• Assesses needs and conducts educational in-services for providers.
• Manages and documents the admission process for each referral and coordinates with Caregiver Support Center.
• Communicates with physicians and hospital staff to coordinate care plans and follow up on patient status.
• Ensures timely arrangements for equipment and services and maintains patient safety and confidentiality.
