Job summary

Horizon Hospice Care in Michigan seeks a Patient Support Specialist to deliver compassionate patient services and manage referrals through Start of Care or Resumption of Care. You will respond to inquiries, coordinate with intake and clinical teams, and help ensure timely, compliant processing of referrals.

The role emphasizes teamwork, attention to documentation, and ongoing collaboration with multiple departments to support quality patient care and a positive patient experience.

Qualifications

• Bachelor's degree in healthcare administration, marketing, business, or related field preferred.
• Minimum of 1 year of experience in healthcare sales, marketing, or community relations, preferably in hospice or healthcare services.
• Strong interpersonal and communication skills, with the ability to establish and maintain relationships with diverse stakeholders.
• Knowledge of hospice regulations, Medicare/Medicaid guidelines, and healthcare industry trends is desirable.
• Valid driver's license and reliable transportation required.

Responsibilities

• Provide a team approach to building Horizon Hospice Care's reputation of quality service, dependability, and ownership of delivering great care to our patients, sources, and staff.
• Read and respond professionally to emails and phone calls in a timely and effective manner.
• Receive inbound phone calls and address questions/needs professionally and comprehensively.
• Conduct extensive clinical review for all incoming referrals prior to Start of Care or Resumption of Care.
• Document all pertinent information with regards to all clinical documents received and reviewed.
• Coordinate with Intake, Clinical Leadership, Scheduling, Revenue Cycle, Marketing & Management to process referrals.
