Job summary

Heritage Behavioral Health Center- HBHC in Decatur, Illinois is seeking an Access Navigator to support coordination, documentation, and administrative operations for recovery and community-based services. You will help with intake, consent, and cross-team communication to ensure timely care transitions.

The role emphasizes accurate documentation, data collection for grants and reporting, and collaboration across departments to support high-quality behavioral health services.

Qualifications

• High school diploma or equivalent required.
• Strong organizational and time-management skills.
• Experience with electronic health record systems preferred.
• Ability to handle sensitive information with privacy and professionalism.

Responsibilities

• Scan, upload, and organize client documents and tracking within the health record.
• Obtain medical records and signatures and releases of information.
• Support clients through admission, intake, and consent processes.
• Coordinate handoffs between programs to ensure continuity of care.
• Collect and track data for grants, compliance, and quality improvement measures.
• Schedule staffing, meetings, and team calendars; manage day-to-day coverage.
