Qualifications

Responsibilities

• Coordinate medical, dental, and psychiatric appointments.
• Transport residents to appointments and outings.
• Communicate with physicians, medical offices, families, and service providers.
• Support residents with daily living skills and psychiatric symptoms as needed.
• Help residents access appropriate community and healthcare services.
• Monitor and document resident progress and appointment outcomes.
