Job summary

RHA Health Services is seeking a CAP Nurse Case Manager (Part-Time RN) to provide basic case management for medically fragile children in their home environment across Watauga, Avery and Burke counties, with flexible hours and a hybrid work arrangement in Spruce Pine, NC. Coordinate care with the RN and a multidisciplinary team to promote independence and safety.

The role requires a Bachelor’s degree in social work or human services and related case management experience.

Qualifications

• Bachelor’s degree in social work or human services with related experience.
• Directly related case management experience preferred in human services.
• Ability to coordinate services with the RN and interdisciplinary team.

Responsibilities

• Provide basic case management to medically fragile children at risk of institutionalization.
• Coordinate services with the RN to ensure quality care in the client’s home.
• Develop person-centered plans and arrange waiver supports, transportation, and education.
