Mercy House is hiring a Housing Solutions Case Manager in Santa Ana, CA, to support individuals who are homeless or at risk of homelessness under the CalAIM program. You will assess needs, develop housing plans, and advocate for clients across shelters, communities, and homes.

The role emphasizes trauma‑informed care, collaboration with partners, and strong documentation to promote housing stability and self‑reliance.

Qualifications

• Bachelor’s degree in Social Work or related field preferred.
• Two years of direct life experience working with homeless, low‑income, and diverse populations accepted.
• Experience in recovery, mental health, and co‑occurring disorders preferred.

Responsibilities

• Maintain a caseload of 17–25 individuals with regular housing support planning.
• Meet clients in shelters, community sites, and homes; use harm reduction.
• Conduct health and safety visits and document case notes and plans.
• Coordinate with providers, landlords, and housing authorities.
• Assist with housing searches, applications, and move‑in readiness.
• Provide early identification and intervention for housing stability risks.
• Advise clients on life skills and relationships with landlords.
