Job summary

Mercy House, Inc. seeks a Housing Solutions Case Manager under CalAIM to conduct ongoing needs assessments, develop housing support plans, and coordinate navigation services for homeless or at-risk individuals in Santa Ana.

The role requires building partnerships with shelters, health care networks, landlords, and social workers, while applying trauma-informed care and motivational interviewing. Responsibilities include maintaining documentation, managing a 17–25 case load, organizing housing

Qualifications

• Bachelor’s degree in Social Work or related field preferred.
• Experience in recovery, mental health, and co-occurring disorders.
• Ability to work with diverse populations and handle crises.

Responsibilities

• Maintain a caseload of 17–25 individuals and develop housing support plans.
• Meet clients in shelters, in the community, and in their homes.
• Use harm reduction and trauma-informed approaches in services.
• Collaborate with community providers, health care networks, and landlords.
• Maintain accurate documentation, case notes, and housing records.
• Assist with housing searches and applications, and ensure move‑in readiness.
