Job summary

HYC, INC. in Long Beach, CA is hiring a Housing Case Manager to work directly with program participants to develop, arrange, and coordinate housing-focused case management services under the CalAIM Community Supports Program.

The role focuses on housing navigation for adults and children, helping clients access benefits, secure housing, and maintain stability, with coordination to mental health services, employment resources, and landlord relationships.

Qualifications

• Bachelor's degree in social services preferred; high school diploma required.
• One year's experience working with individuals experiencing homelessness, mental health issues, substance use disorders, and physical disabilities
• Permanent U.S. residency or citizenship
• Ability to pass criminal clearance background check and TB screening
• Valid California state driver's license, current auto insurance, and a good driving record

Responsibilities

• Develop and implement individualized service plans tailored to each client's needs, including addressing barriers and setting goals.
• Advocate for clients to ensure benefits and resources are obtained and maintained.
• Provide housing placements, referrals, and connections to mental health and other services as needed.
• Maintain accurate documentation of all services and contract requirements.
• Coordinate transportation and accompany members to critical appointments as needed.
