Job summary

Stepping Up Santa Cruz is hiring a CalAIM Clinical Case Manager to coordinate comprehensive clinical care for ECM, CS, and CHW programs. You will consult with Case Managers, engage with members in the community, and support those with complex behavioral health and medical needs.

The role blends office-based coordination with field visits, requiring initiative and a compassionate approach to improve lives. A California license and valid driver’s license are required.

Qualifications

• Active California MFT, LCSW, or LPCC license.
• Minimum two years of clinical experience with complex needs.
• Experience with crisis intervention, assessment, and care planning.
• Valid California Driver’s License and own vehicle.

Responsibilities

• Conduct biopsychosocial assessments and clinical formulations.
• Develop individualized care plans with Trauma‑Informed Care and Harm Reduction.
• Coordinate with Case Managers to support complex behavioral health needs.
• Document services in the EHR per Medi‑Cal and CalAIM standards.
• Provide clinical guidance on member engagement and discharge planning.
