Job summary

Methodist Le Bonheur Healthcare in Memphis seeks a Social Worker (MSW) to support safe, timely patient transitions across the acute care episode. You will conduct psychosocial assessments, develop discharge plans, coordinate post-acute services, and help families navigate available resources.

This non-clinical role emphasizes collaboration with the interdisciplinary team, patient advocacy, and guidance on community resources, Medicaid applications, and care planning.

Qualifications

• Master's Degree in Social Work required.
• Licensed Master Social Worker TN or eligible within 12 months.
• Experience in acute care, healthcare, social services or community resource coordination preferred.
• Knowledge of discharge planning, community resources, and healthcare systems preferred.
• Strong communication and interpersonal abilities; compassionate patient advocacy.

Responsibilities

• Discharge Planning & Care Coordination: psychosocial assessments, individualized discharge plans, post-acute service coordination, and family communication.
• Patient & Family Support: education on resources, advance care planning discussions (non-clinical), support for psychosocial stressors, patient rights advocacy.
• Resource Navigation & Community Coordination: identify needs, connect to community agencies, coordinate with insurance case managers, assist Medicaid/applications.
• Interdisciplinary Collaboration: participate in rounds and meetings, communicate needs to care team, assist with complex cases.
• Documentation & Compliance: maintain EMR documentation, adhere to CMS/JCAHO policies, support readmission reduction initiatives.
