Job summary

Baystate Health in Massachusetts seeks a licensed social worker to assess and address the psycho-social needs of patients and families, helping them access medical care and supportive services to achieve their health goals. You will identify patients who need discharge planning and provide counseling and crisis intervention.

You will collaborate with staff and community providers to coordinate referrals, ensure smooth transitions, and support patients across care settings.

Qualifications

• Bachelor's degree in Social Work from a CSWE-accredited school.
• Two years of related professional experience required.
• Fieldwork placements may substitute for one year of experience.
• Valid social work license from the Commonwealth of Massachusetts, minimally at the LSW level.

Responsibilities

• Assess and address psycho-social needs of patients and families.
• Identify patients requiring discharge planning services.
• Provide counseling, crisis intervention and support to patients, and ensure referrals to post-hospital care and services.
• Coordinate with staff and with community-based providers as required.
