Qualifications

• Master’s of Social Work or Bachelor’s of Social Work with Master’s Degree in related field.
• Current American Heart Association Basic Life Support Healthcare Provider Completion Card upon hire and maintained.
• One year of previous social work experience.
• Basic computer skills including knowledge of MS Office – Outlook and Word.
• Knowledge of healthcare economic issues impacting acute care.

Responsibilities

• Coordinate referrals for post-acute services in a timely manner.
• Communicate with patient/c caregiver and physician on payer or financial issues.
• Coordinate and document complex discharge plans and options.
• Participate in care conferences/ethics consults as indicated.
• Provide counseling and research placement options for substance abuse and mental health.
