Job summary

Bond Health in Brooklyn, NY is seeking Direct Support Professionals for a part-time role. Hours: Monday through Friday, 3:00 p.m. to 6:30 p.m., provide person-centered support to individuals with ID/DD, promote independence, and help participants participate in the community.

You will monitor well-being, maintain confidentiality, advocate for rights, document progress, and collaborate with the team to ensure safe, respectful care in accordance with agency policies.

Qualifications

• HS diploma and experience working with individuals with ID/DD.
• Excellent written and oral communications skills.
• Flexibility in scheduled work hours as needed.
• Eager to learn and hone skills to be a professional in the field.
• Able to work with a diverse group of people and be a team player.

Responsibilities

• Assist individuals receiving services in leading self-directed lives and to participate fully in their local communities.
• Support emotional, physical, and personal well-being while reducing risk of harm.
• Respect privacy and confidentiality, breaking it only if imminent harm exists.
• Promote human and civil rights and help individuals understand their rights and responsibilities.
• Advocate for inclusion and community participation and educate in age-appropriate communications.
• Ensure services align with agency policies and regulatory guidelines.
• Identify and report suspected neglect or abuse and participate in investigations as needed.
• Know each individual's Habilitation plan and goals and implement plans as written.
• Document data via EMR, monitor progress, and support lifelong learning and growth.
• Provide a therapeutic, professional environment and maintain appropriate conduct.
• Communicate with family, service coordinators, and others as directed.
• Provide the appropriate level of support and supervision and engage individuals in programs and activities.
• Assist in daily living activities as needed (eating, toileting, dressing, hygiene, etc.).
• Attend meetings and trainings as requested and travel with individuals to approved activities.
