Qualifications

• High school diploma, HSE or GED is required.
• A minimum of 2 years' social services experience is required.
• Bi-lingual Spanish/English is preferred but not required.
• Experience with electronic case management systems (ETO) is preferred.

Responsibilities

• Collaborate with residents in developing client-centered, recovery-oriented service plans.
• Provide integrated counseling and case management to promote health, wellness, and housing stability.
• Utilize a harm reduction approach for diverse caseloads.
• Make referrals and liaison with care coordination agencies and health providers.
• Engage residents through apartment visits, outings, and appointment escorts.
