Job summary

Jericho Project is seeking a Tenant Service Coordinator in the Bronx to provide case management for a 79-unit Permanent Supportive Housing program. Location: Gun Hill/The Laconia; reports to the Program Director.

Compensation ranges from $53,000 to $55,000 yearly for this non-exempt role. The role requires a Bachelor's in human services, experience with homelessness or related fields, weekend availability, and strong organizational, writing, and communication skills.

Qualifications

  • A Bachelor's degree in human services or related field required.
  • Experience with homelessness, substance use, mental health and/or trauma settings preferred.
  • Must be willing to work a weekend day.
  • Strong organizational, writing, communication, and computer skills.

Responsibilities

  • Maintain a caseload of high risk/high intensity tenants.
  • Provide enhanced case management using Housing First and MI techniques.
  • Conduct assessments and program intakes for residents.
  • Develop and update person-centered service plans every 6 months.
  • Coordinate with the psychiatrist and manage appointment times.
  • Provide referrals and linkages to public benefits and community resources.
  • Coordinate employment and educational assistance with Workforce Opportunities.
  • Complete monthly Audit Tool and ensure citations cured.
