Job summary

Jericho Project is seeking a Case Manager to provide residential and long‑term support to homeless and at‑risk adults transitioning into permanent housing from a Bronx‑based location.

You will carry a caseload of approximately 20 to 25 formerly homeless and/or low income individuals, with regular contact and thorough documentation within an electronic health record, coordinating services and supports toward housing stability.

Qualifications

• Bachelor’s degree in human services or related field.
• Strong organisational, writing, communication, and computer skills.
• Some experience working with low‑income or homeless populations.
• Fluent in Spanish is strongly preferred.
• Candidates must be self‑ starters with community work energy.

Responsibilities

• Provide direct case management services on-site and in the field, including mental health and substance use assessments and developing service plans.
• Use evidence‑based models such as Motivational Interviewing, Critical Time Intervention and Harm Reduction.
• Maintain tenant files/charts in the electronic health record and document outcomes.
• Coordinate and document clinical case management and psychosocial services; develop and review Individual Service Plans.
• Serve as liaison between tenants and community providers; facilitate life skills and education groups.
• Complete referrals to medical and mental health providers and assist with employment goals where applicable.
• Ensure compliance with organisational and funder regulations and complete required documentation.
• Coordinate aftercare services and post-discharge follow‑up; conduct outreach and engagement.
