Job summary

Jobtailor is seeking a hybrid, full-time program coordinator in Tennessee to support Medicaid-compliant service delivery and client advocacy. You will develop care plans, train clients, and coordinate plan submissions while maintaining thorough documentation.

Responsibilities include audits, reporting, stakeholder collaboration, and special projects. A related degree with experience or a combination of degrees and years of work with individuals with disabilities is required.

Qualifications

  • A four-year degree in Human Services and at least one year of experience with the listed populations.
  • A four-year degree in any subject and two years’ experience with the listed populations.
  • A two-year degree in Human Services and five years’ experience with the listed populations.

Responsibilities

  • Apply a person-centered approach to client and program functions.
  • Coordinate day-to-day implementation of contractual requirements within Medicaid guidelines.
  • Create care plans based on client needs.
  • Train clients on services, timecard submission, and program roles.
  • Audit services and report findings to the contracted entity.
  • Assist with plan and budget development, submissions, and revisions.
  • Provide service to clients, employees, third-party organizations, and MCOs.
  • Support daily operations and client advocacy.
  • Collaborate with stakeholders regarding service delivery.
  • Conduct advanced reporting and oversight activities.
  • Lead, supervise, and complete special projects, including care-plan development and implementation.
  • Market services to referral sources.
  • Represent the company at stakeholder health and provider fairs.
  • Enroll clients and employees in services, including plans and budgets.
  • Assist clients with program eligibility requirements.
  • Travel to client homes for meetings.
  • Conduct client assessments and contacts.
  • Maintain documentation in the CRM database.
  • Compile information for auditors.
  • Supervise deliverables and ensure timely project completion.
  • Comply with legal requirements and procedures.
  • Perform other duties as assigned.

Requirements
  • A four (4) year degree in Human Services and at least one (1) year professional experience working with elderly people, adults with physical disabilities, individuals with intellectual disabilities, or persons with developmental disabilities; or
  • A four (4) year degree in any other subject and two (2) years professional experience working with the listed populations; or
  • A two (2) year degree in Human Services and at least five (5) years professional experience working with the listed populations
  • Must have reliable transportation
  • If driving, a valid driver's license and auto insurance are required
  • Must provide own reliable Internet service for the hybrid position
  • Legally authorized to work in the United States
  • Comfortable with public speaking
  • Effective communication, problem-solving, and decision-making skills
  • Computer efficiency
  • Ability to prioritize and multitask
  • Bilingual preferred in various locations
