Job summary

Qualifications

  • Facilitate and lead an ISP team.
  • Preferred experience with intellectual and/or developmental disabilities in a community-based setting.

Responsibilities

  • Help people with developmental disabilities identify and obtain services and supports.
  • Advocate for, monitor, and evaluate services with individuals, families, or guardians.
  • Manage a caseload of varying ages, disabilities, and complexity.
  • Build trusting relationships through regular contact and planning.
  • Identify needs and create action plans.
  • Coordinate federal, state, county, and community programs.
  • Prepare Utilization Review requests and service authorizations.
  • Monitor and assess the quality, timeliness, and effectiveness of services.
  • Document all activities in written case notes within three business days.
  • Maintain a minimum of 120 TCM billable hours per month.
  • Complete mandated reporting protocols for suspected abuse or neglect.

Job description

  • Help people with developmental disabilities and their families identify and obtain needed services and supports
  • Advocate for, monitor, and evaluate services with individuals, families, or guardians
  • Manage a caseload of varying ages, disabilities, and complexity
  • Build trusting relationships through regular contact and planning
  • Identify needs and create action plans
  • Develop community relationships and support systems
  • Research resources, develop new resources, and make referrals to collaborating agencies
  • Prepare Utilization Review requests and service authorizations
  • Coordinate federal, state, county, and community programs
  • Monitor and assess the quality, timeliness, and effectiveness of services
  • Conduct monitoring by phone, email, and in-person meetings monthly, quarterly, and annually
  • Complete mandated reporting protocols for suspected abuse or neglect
  • Document all activities in written case notes within three business days
  • Maintain a minimum of 120 TCM billable hours per month
  • Complete assigned training and participate in continuing education, conferences, and seminars
  • Carry out special assignments or duties
Requirements

  • Ability to meet timelines accurately and return calls/emails within one business day
  • Ability to work independently and be resourceful
  • Mathematical skills for understanding, reviewing, creating, and maintaining budgets
  • Ability to handle confidential information discreetly
  • Ability to advocate for individuals' rights
  • Ability to facilitate and lead an ISP team
  • Preferred experience working with individuals with intellectual and/or developmental disabilities in a community-based setting or related fields
  • Preferred experience with case management and documentation applications such as Setworks, Kronos, and Therap
  • Preferred multidisciplinary team experience with person-centered plans
  • Preferred experience with Microsoft Office Suite
