Job description

  • Manage an assigned caseload of adult and pediatric members
  • Maintain a caseload of up to 50 members and conduct monthly in-person visits
  • Conduct comprehensive physical, emotional, and social needs assessments
  • Develop and tailor individualized care plans based on assessments, medical history, preferences, needs, and goals
  • Coordinate communication among healthcare providers, social workers, therapists, and other care team members
  • Collaborate with medical doctors, clinical consultants, housing navigators, and leaders
  • Monitor member progress and update care plans according to policy and compliance requirements
  • Ensure prescribed treatments and interventions are followed
  • Communicate significant member concerns and status updates to PCPs and specialty care providers
  • Provide member service through telephone and in-person support
  • Maintain accurate records of assessments, care plans, and member interactions
  • Ensure compliance with relevant regulations and standards
  • Complete required documentation accurately and on time
  • Provide leaders with case progress updates
  • Advocate for members and help them navigate healthcare, treatment options, and services
  • Educate members and families about health topics, treatment options, and self-care
  • Connect members with community resources, support services, housing assistance, financial aid, and counseling
  • Plan and coordinate discharge transitions from hospitals or long-term care facilities
  • Participate in training new employees
  • Perform other duties as assigned or required by departmental policy
Requirements
  • Diploma/GED required; associate degree preferred
  • Minimum 1+ years of experience in healthcare, social services, or care coordination
  • Current and valid driver’s license with a clean driving record
  • Reliable transportation and proof of auto insurance
  • Current BLS certification from the American Heart Association upon start date
  • Fluent in English, written and verbal
  • Bilingual in Spanish
  • Ability to communicate clearly in person, by phone, and electronically
  • Adequate hearing and vision, with corrective devices if necessary
  • Ability to identify problems and use logic to develop and implement solutions
  • Excellent organizational skills and attention to detail
  • Commitment to patient confidentiality and ethical healthcare standards
  • Ability to lift, carry, push, or pull up to 20-25 pounds
  • Ability to climb stairs and navigate uneven terrain
  • Ability to bend, reach, and conduct visits in non-traditional environments
  • Ability to remain stationary and move around the office or travel throughout the community
  • Ability to operate a vehicle and travel to meet members
  • Ability to maintain professionalism and safety in diverse environments
  • Ability to work independently and complete assignments within established policies and procedures
  • Competence with computers, email, virtual platforms, and Microsoft Office-based programs
  • Ability to type for extended periods
