Qualifications

• Bachelor's degree in human services or related social sciences.
• Must maintain a valid PA Driver's license.
• Experience with case management and community-based services is preferred.

Responsibilities

• Provide case management to seriously and persistently mentally ill clients in community settings.

• Advocate for clients and coordinate services across providers.
