Job summary

Center for Community Resources in Williamsport, PA is seeking a Blended Case Manager for a full-time role. You will provide case management services to individuals with serious mental illness or emotional disturbances and work with families to access, coordinate, and monitor needed services.

The position requires a Bachelor’s degree in Human Services or related field and at least one year of MH case management experience.

Qualifications

• Bachelor’s degree in Human Services or a related field.
• One year experience in MH Case Management.
• Act 33/34 and FBI clearances required.
• Valid driver's license.

Responsibilities

• Provide blended case management services to individuals with mental health diagnoses.
• Complete reports and documentation timely.
• Participate in 24-hour on-call rotation.
• Maintain confidentiality and quality standards.
