Job summary

Qualifications

• Bachelor's degree with major course work in sociology, social welfare, psychology or related field or RN.
• High school diploma plus 12 semester credits in related field and two years’ experience with mental health consumers.
• Any Blended Case Manager who has served since 1989 and remained employed with a high school diploma.

Responsibilities

• Complete a strength-oriented assessment within one month of opening a case.
• Enter case notes within five working days.
• Coordinate care with all community providers involved with the consumer.
• Promote consumer growth towards recovery and community tenure.

Education

Bachelor's degree in sociology/social welfare/psychology

Registered nurse (RN)

High school diploma + 12 college credits in related field

Direct contact with mental health consumers (2+ years)

Valid driver’s license
