Job summary

Community Guidance Center is seeking Blended Case Managers to support up to 30 consumers, conducting home or community visits as needed and coordinating with the treatment team to implement service plans.

Position requires a Bachelor’s degree or equivalent in a relevant field, plus strong organizational, communication, and crisis-management skills. Full-time and part-time options available with on-site responsibilities in the Dubois area.

Qualifications

• Bachelor’s degree or equivalent with relevant field.
• High school diploma with five years of related mental health direct care experience.

Responsibilities

• Assess and plan services, setting goals with consumers.
• Link with services and access to resources per service plan.
• Monitor service delivery and address issues in collaboration with the consumer.
• Provide 24/7 on-call coverage in crisis situations.
• Document thoroughly and maintain regulatory compliance.
• Attend supervision and training as required.
