Job summary

Dickinson Center, Inc. in Ridgway, PA seeks a Blended Case Manager to coordinate services for individuals with mental health diagnoses in Elk and Cameron Counties. You will provide wellness coaching and work closely with the Wellness Nurse to ensure compliant, integrated care.

The role requires an on‑call commitment and may include Administrative Case Management as caseload needs evolve. A valid driver’s license is required to support community-based activities.

Qualifications

• Bachelor's degree in human services or related field.
• RN credential or the equivalent licensure is acceptable.
• High school diploma with 12 college credits in human services and two years’ direct experience with mental health consumers.

SUMMARY

This position provides monitoring, linking and coordination of services to individuals with a mental health diagnosis, in Elk and Cameron Counties. Provides wellness coaching to designated consumers along with coordinating these activities with the Wellness Nurse. Ensures adherence to state and provider regulatory standards. This position also participates in an on-call schedule. This position may also provide Administrative Case Management services to individuals on their caseload as those consumers no longer require the intensity of the Blended Case Management program.

POSITION QUALIFICATIONS

Education / Experience

• Bachelor's degree with major course work in human service, education or social science field; or
• Registered Nurse; or
• A high school diploma and 12 semester credit hours in human service, education or social science AND two years of experience in direct contact with mental health consumers.

Certifications & Licenses

• Valid driver's license with clean driving history
