Job summary

City of Tuscaloosa seeks a Tuscaloosa Police Department Behavioral Intervention Team (TPD BIT) case manager to aid with a growing caseload of at-risk individuals. The role involves regular home visits, needs assessment, and reporting on families visited to address unmet medical, mental health, or social needs.

Qualifications

• Bachelor’s degree in social work (BSW) required; valid driver's license.
• One year of case management experience preferred.

Responsibilities

• Contacts and assesses potential BIT Case Management subjects and identifies barriers to care.
• Maintains contact with subjects, monitors progress and assists in goal setting.
• Documents all subject and agency encounters within 24 hours of service.
• Provides weekly reports to the BIT Case Coordinator and assists in monthly activity reports.
• Builds relationships with community and City agencies/entities.
• Sets up social work hubs in target areas in coordination with TFR EMS Prevention.
• Demonstrates compassion, relates to subjects with various needs, and motivates treatment plan adherence.
• Communicates treatment plans clearly to subjects, families, and professionals.
• Maintains strong organizational and documentation skills; manages multiple cases.
