Job summary

AltaPointe Health is seeking a dedicated Case Manager to support individuals across mental health and related services in the Sylacauga area. You will travel to consumers' residences or schools to monitor well-being, empower clients and families, and help sustain progress after case management ends.

Responsibilities include assessing needs, linking to resources, documenting plans, and communicating with the treatment team. A Bachelor's degree in a behavioral health field and driving are required.

Qualifications

• Bachelor’s degree in a behavioral health or social services related discipline preferred.
• Must meet all driving requirements.

Responsibilities

• Case managers assess consumer needs and plan goals documented in ISP.
• Provide linkage to community resources and monitor progress.
• Advocate on behalf of the consumer to secure services and resources.
• Assist with transportation needs to psychiatric appointments.
• Document SUN-R assessments and progress notes in a timely manner.
