• Draft service plans and review progress for potential closure.
• Deliver services including crisis intervention, advocacy, transportation, budgeting, housing, and referrals.
• Assist clients to wellness and autonomy through counseling and care coordination.
• Document all clinical activities meeting criteria and standards.
