Job summary

Seneca seeks a bilingual Wraparound Facilitator to lead family-centered planning meetings and coordinate intensive mental health support for children and families with complex needs. The role emphasizes assessment, team leadership, and documented treatment planning in partnership with county and community agencies.

Responsibilities include case management, crisis response, and collaboration with social workers, probation officers, therapists, and teachers.

Qualifications

• Bachelor's degree + 4 years' experience working with children/youth and families in a mental health setting.
• Willingness to respond to crisis situations and be part of rotating emergency on-call system.
• Flexible schedule with ability to work evenings and weekends.

Responsibilities

• Facilitate Child and Family Planning Meetings, including meeting prep, coordinating attendance, and documenting notes within two days.
• Case management and coordination of care.
• Collaborate with county and community partners (social workers, probation officers, therapists, teachers).
• Complete mental health notes, Family Team Meeting notes, Wrap Action Plan, and incident reports.
• Intervene with clients' behavior management programs and provide crisis management as needed.
• Attend team meetings, trainings, supervision, and adhere to clinical best practices.
• Complete clinical documentation for MHRS credentials, including assessments and treatment planning.
