Qualifications

• Master’s degree in social work, psychology, counseling, rehabilitation, nursing gerontology or sociology with 1 year experience.
• Bachelor’s degree with 2+ years experience with the population(s).
• RN or Associate degree with 3+ years experience with the population(s).
• Successful criminal background checks per CA and federal requirements.

Responsibilities

• Coordinate daily operation of Medicaid Waiver services.
• Promote participant independence and personal growth.
• Maintain accurate documentation per state and agency requirements.
• Build relationships with agency staff, service providers, and families.
• Facilitate onboarding and access to community resources for participants.
• Participate in planning, meetings, and program evaluations.
• Travel to events, conferences, and related activities.
