Job summary

El Futuro is seeking a full-time Case Manager for Substance Use in Durham, NC. The role delivers culturally and linguistically responsive care coordination through the Sin Ataduras program, supporting Latino individuals and families with behavioral health and community-based services.

The position involves 37.5 hours weekly in a hybrid schedule, one in-person Durham clinic day, and Friday mornings meetings; evenings and occasional weekends are required.

Qualifications

• Bachelor's degree in social work, psychology, human services, public health, or a related field.
• Experience providing case management, care coordination, resource navigation, or other human services.

Responsibilities

• Serve as a primary contact for DWI program clients, ensuring warmth, clarity, cultural responsiveness, and confidentiality.
• Coordinate appointments for assessments, intakes, groups, transfers, and discharges; monitor follow-up needs and barriers.
• Provide timely reminders for appointments, group sessions, required documents, and client payments.
• Guide clients in using Zoom, DocuSign, patient portal, and other digital tools; troubleshoot access issues.
• Document actions taken and escalate urgent matters to appropriate staff.
