Job summary

Alignment Health is seeking a Registered Nurse for care management, primarily telephonic, to assess, develop and coordinate care plans for members with complex and chronic needs, across the care continuum.

Requires minimum 5 years of clinical case management, an RN license in the applying state, and an associates or bachelor’s degree in nursing (BSN preferred). Compensation range varies by location and experience.

Qualifications

• Minimum 5 years of clinical case management experience.
• Associates or Bachelor’s degree in Nursing required; BSN preferred.
• Current, unrestricted RN license in the state you are applying to.
• Strong communication, leadership and collaboration skills.

Responsibilities

• Ensure member access to services appropriate to health needs.
• Identify, assess, and manage members per established criteria.
• Facilitate care plan authorizations/referrals within benefits structure or through extra-contractual arrangements.
• Monitor and evaluate effectiveness of the care management plan.
• Interface with clinicians to develop care management treatment plans.
