Alignment Health is seeking a Remote RN Case Manager SNP to coordinate care for members with complex and chronic health needs. The role requires an active California RN license (Non-Compact) and focus on closing gaps in care, education, and effective communication with families, PCPs, and providers.

You will work remotely with a cross-state licensure plan, supporting care management across a broader network while maintaining PHI confidentiality and adhering to policies.

Qualifikationen

• Active RN license in California (Non-Compact).
• 1 year or more of case management experience highly preferred.
• Understanding of health plan processes and care coordination.
• Strong written and verbal communication across diverse stakeholders.
• Willingness to obtain licensure in additional states as needed.
