Job summary

Alignment Health is seeking an RN with extensive clinical case management experience to coordinate care for seniors with chronic needs. This role is primarily telephonic and requires strong communication, leadership, and problem‑solving skills to develop and monitor comprehensive care plans.

The ideal candidate will hold an RN license in their state, possess a Bachelor's or Associate degree in Nursing, and have a track record of delivering high‑quality, cost‑effective care.

Qualifications

• Minimum 5 years of clinical case management experience or equivalent education/experience.
• Associates or Bachelor's degree in Nursing; BSN preferred.
• Current, unrestricted RN license in the state for which you are applying.

Responsibilities

• Assess, develop, implement, coordinate, monitor, and evaluate care plans for members with complex and chronic care needs.
• Coordinate and monitor member progress to ensure cost-effective care that complies with policy and regulations.
• Interface with Primary Care Physicians, Hospitalists, Nurse Practitioners, and specialists to develop care plans.
• Counsel and engage with patients and families on available care options.
