Job summary

Alignment Health is seeking a Bilingual RN Case Manager for SNP remote work in the United States. The role requires a California RN license (Non-Compact) and offers full-time hours with a schedule of 8:00am - 5:00pm Pacific Time.

You will manage care for seniors with complex needs, coordinate with care teams, and help close gaps in care. The ideal candidate has 2+ years of clinical and 1+ year in case management, with strong communication skills and proficiency in Microsoft Office.

Qualifications

• Must hold an active California RN license (Non-Compact).
• Minimum 2 years of clinical experience and 1 year in case management.
• Willingness to obtain additional licenses where required (Nevada, Arizona, etc.).

Responsibilities

• Coordinate care as a resource for the member, family, and physician.
• Ensure access to appropriate care and facilitate referrals/authorizations.
• Complete comprehensive assessments within scope of practice.
• Collaborate with PCP and care team to implement a plan of care.
• Interface with physicians and specialists for care management plans.
• Provide education and self-management support to members.
• Assist in problem solving with providers, claims, or service issues.
• Work with delegated or contracted providers to assure quality care coordination.
• Maintain confidentiality of all PHI per laws and policy.
