Job summary

The role requires RN/LPN licensure, preferred managed care experience, and bilingual English/Spanish abilities. The Nurse Case Manager will participate in case conferences, collaborate with physicians, document and review care plans, and advocate for patient access to services.

Qualifications

• RN or LPN licensure is required.
• Experience in managed care is preferred.
• Bi-lingual English and Spanish is preferred.

Responsibilities

• Be the system navigator and point of contact for enrolled patients.
• Provide enhanced care coordination services addressing health system barriers.
• Identify and link patients with cultural and community resources.
• Collaborate with other service providers for care coordination.
• Work with patients by phone to review plan of care and progress.
• Conduct outreach to assigned members to promote program engagement.
• Perform other duties as required by manager.
