Job summary

Alignment Health is seeking a Remote RN Case Manager SNP to oversee care for members with complex chronic conditions. You will work with the member, family, PCP, and care team to implement and monitor a comprehensive plan of care, ensuring timely access to services.

The role requires an active CA RN license (Non-Compact), strong communication, and the ability to coordinate across multiple providers. Expect a 8:00am–5:00pm PT shift and remote work from home.

Qualifications

• Licensed RN in California (Non-Compact) is required.
• AS in Nursing is required; BSN preferred.
• Experience with case management and health plan processes highly valued.

Responsibilities

• Coordinate care for members and families.
• Facilitate referrals and authorizations within benefit structure.
• Complete comprehensive health assessments within scope of practice.
