Qualifications

• Must have active RN license in California (Non-Compact) and be willing to obtain additional state licenses.
• Experience in case management and care coordination for chronically ill or frail populations preferred.
• Strong communication and problem-solving skills with ability to educate and support members.

Responsibilities

• Coordinate care for members with complex needs as a resource for members, families, and physicians.
• Facilitate referrals/authorizations within the benefit structure to ensure timely access to care.
• Perform comprehensive assessments of health status, utilization, and care plans, and collaborate with the care team.
