SCAN Health Plan is seeking a bilingual Community Health Worker to support seniors in the South Bay by coordinating care, visiting members in person or by phone, and guiding them through the healthcare system. You will partner with the care team to implement personalized plans, address barriers, and empower members to age in place.

Requirements include a High School Diploma (or equivalent) with a Community Health Worker Certificate preferred, 1+ year of engagement experience, and a valid

Formación

• BILINGUAL required in English/Spanish.
• Travel 50+% of the time in the assigned area.
• Must have a valid driver’s license and reliable transportation.
• 1+ years of Community Engagement Experience.
• Experience with managed care or case management preferred.

Responsabilidades

• Assist frail seniors to age in place and navigate care systems.
• Conduct in-person or telephonic visits to implement care plans and coordinate resources.
• Perform assessments for chronic conditions and psychosocial needs.
• Provide coaching, education and advocacy to members and families.
• Collaborate with interdisciplinary teams and document care plans.
