Job summary

SCAN Group is seeking a Community Health Worker to enhance frail seniors' ability to age in place and navigate the healthcare system. You will work telephonically and in-person to implement plans of care, advocate for members, and provide member-centric interventions.

Responsibilities include conducting assessments, addressing barriers to care, coordinating with care teams, and supporting families through referrals and resources.

Qualifications

• High School Diploma or equivalent experience.
• Community Health Worker Certificate strongly preferred.
• Bilingual English/Spanish required.
• Travel 50%+ in the assigned area; valid drivers license and reliable transportation.
• 1+ years of Community Engagement Experience.
• Experience with managed care or healthcare environment preferred.
• 1+ years working with seniors, home visits, and remote work preferred.
• Basic technical skills for the role; strong communication.

Responsibilities

• Collaborate in general assessments to identify chronic conditions and psychosocial challenges affecting independent living.
• Conduct face-to-face or phone visits to address care barriers and support primary and preventive care.
• Coordinate care and empower members and families in managing chronic conditions through coaching and navigation.
• Perform Enhanced Care Management activities for specified populations of focus.
• Implement personalized care plans tailored to high-risk members and follow up after discharge.
