Job summary

East Bay Innovations (EBI) is hiring a Case Manager I to coordinate and support clients transitioning from facilities to their communities in Alameda County. The role includes helping secure IHSS, affordable housing, and community services, while maintaining detailed client records.

The position requires two years in human services, a BA or equivalent experience, and Spanish fluency with a language exam. Travel across Alameda County is involved; English language proficiency is essential.

Qualifications

  • Two years of experience in human services; IHSS and Medi‑Cal preferred.
  • Bachelor’s degree required; education can be substituted with an additional 2 years of field experience.
  • Bilingual in Spanish and must pass a language proficiency exam.

Responsibilities

  • Coordinate and support clients to transition safely from institutional care settings to the community; create a comprehensive service plan.
  • Assist clients to access IHSS, affordable housing, and community health services.
  • Maintain accurate records, including state and program required documentation, case notes, billing information, and client files.
  • Facilitate and assist client with developing a circle of support with family, friends, and neighbors.
  • Act as liaison to clients, their support systems, landlords, employers, and other agencies to maintain living options.
