Job summary

East Bay Innovations is seeking a Case Manager I (TCCM) to support clients transitioning from nursing facilities to community living. The role involves coordinating services, securing IHSS and affordable housing, and guiding clients through benefits, with a focus on bilingual Spanish speakers across Alameda County.

The position requires two years in human services, a bachelor’s degree (or equivalent experience), and passing a language proficiency exam. Travel within Alameda County is expected.

Qualifications

  • Two years of experience in human services; IHSS/Medi-Cal experience preferred.
  • Bachelor’s degree may be substituted with an additional 2 years of relevant experience.
  • Bilingual in Spanish and must pass a language proficiency exam.

Responsibilities

  • Coordinate and support clients to transition safely from institutional care to community living with a comprehensive service plan.
  • Help clients access IHSS, affordable housing and other community health services.
  • Maintain accurate records, documentation, and client files as required.
  • Facilitate a circle of support involving family, friends, and neighbors; liaise with landlords, employers and other agencies.
  • Communicate client information to the direct supervisor promptly and maintain a positive, proactive attitude.

Case Manager I (TCCM)
Our Transitional Care and Case Management (TCCM) department is seeking a Case Manager I to provide coordination and support to clients/family in their transition from nursing facilities/long term residential facilities with the goal of coming back into their community. A Case Manager will support their client with securing affordable housing, navigating through Government benefits and helping locate adequate services like In Home Support Services (IHSS) or even nursing support. While our agency supports individuals with developmental disabilities, the TCCM department works with individuals experiencing homelessness, substance misuse, mental health crisis, difficulty with hygiene, and challenging behaviors.

Additionally, we are seeking someone to support our Spanish speaking clients throughout Alameda County. East Bay Innovations offers Language Stipends for staff based on language fluency assessment and client, department, and agency needs.

Compensation:
  • $28.02 per hour
  • Additional Language Stipends for staff based on language fluency assessment and client/department/agency needs
Schedule:
  • Full-time schedule, 40 hours per week.
  • Monday through Friday, typically between the hours of 8:30AM-5:00PM.
Locations:
  • Traveling and supporting clients throughout Alameda County.
  • Possession of a valid California driver's license, a driving record that meets agency standards, auto insurance, and access to a vehicle.
Qualifications:
  • Two years of experience working in the field of human services; experience working with programs such as IHSS and Medi-Cal preferred.
  • Bachelor’s degree (education can be substituted with additional 2 years of experience in the field).
  • Bilingual in Spanish, and must pass a language proficiency exam.
Duties and Responsibilities:
  • Provide coordination and support to enable clients to transition safely from institutional care settings to the community. Create a comprehensive service plan to ensure ongoing services in the community.
  • Assist clients to access and utilize resources they will need to live in their own home, i.e. IHSS, affordable housing, community health services, Regional Center services.
  • Maintain accurate and detailed records, including State and program required documentation, case notes, billing information, and client files.
  • Facilitate and assist client with the development of a circle of support to build and maintain relationships with family, friends, and neighbors.
  • Work effectively with significant persons in each client’s life, i.e. family members, friends, significant others. Work effectively with other EBI staff, representatives from other agencies, and members of the community.
  • Communicate all information relevant to client support to direct supervisor in a timely manner.
  • Maintain a consistent, positive attitude in order to successfully support each client.
  • Schedule service times to accommodate client needs.
  • Act as liaison to clients, their support systems, landlords, employers, and other agencies to maintain living options.
  • Act as liaison between EBI and other State and Local agencies, i.e. Housing Authority and Social Security.
  • Provide cross-coverage for colleagues as needed.
  • Complete EBI Training curriculum and other required training.
  • Complete EBI shadowing as set forth by supervisor.
  • Participate in staff meetings and ongoing trainings as assigned.
  • Other duties and admin support as assigned.
