Job summary

PATH is seeking a Bi-Lingual Case Manager to join our Interim Housing Services team in San Jose, CA. The role focuses on helping clients break the cycle of homelessness through individualized support plans, housing navigation, and access to health and social services.

Responsibilities include intake, ISP development, referrals, assessments, and maintaining client files. Driving is required; full-time, non-exempt, on-site position with generous benefits and schedule options.

Responsibilities

• Conduct intake and determine client needs to develop ISP with housing goals and income targets.
• Provide referrals and support to access resources aligned with ISP goals.
• Complete program assessments (e.g. VI-SPDAT) and document findings.
• Monitor progress, conduct 90-day reviews, and adjust plans accordingly.
• Maintain confidential records per HIPAA and PATH policies.
• Offer crisis intervention and trauma‑informed services to increase client safety.
