Job summary

Telecare Corporation in Oakland, California seeks a full-time Case Manager III to provide primary support and case management for individuals with mental illness and co-occurring disorders. You will help clients live as independently as possible in their community under direct supervision.

The role requires a bachelor’s degree in Social/Human Services with related experience, or a high school/GED with substantial direct service experience. Shifts are 9:00 AM–5:30 PM, Monday–Friday, on-site.

Qualifications

• Bachelor’s degree in Social Services or Human Services and 1 year of experience OR High School/GED and four years of direct services in Mental Health or community services.
• An Associate's degree may be substituted for two years of experience.

Responsibilities

• Serve as a primary support and case manager for individuals with mental illness and co-occurring disorders, helping them live as independently as possible in their chosen community.
• Under direct supervision, perform case management functions with required education and experience as per position standards.

Education
